How to Book Your Consultation with Dr Jen
(With Checklist)
HOW IT WORKS
To avoid guess work and to ensure personalised results with your health, Dr Jen requires certain test results to help her navigate what you need to secure your health.
Here is an outline of steps required to secure a consult with Jennifer:
STEP 1
☐ BOOK YOUR CONSULTATION
Return to the Online Consultation product page and pay for your consultation.
(The product page is the web page you were previously on before going to this checklist page).
STEP 2
☐ ORDER YOUR FULL DNA GENE TESTING KIT and REPORT
If you need to attain a DNA report you can use SMART DNA, they send their saliva testing kits to most destinations.
You need to enter Dr Jennifer as your practitioner and the report will be sent to her.
Once you have returned the saliva test, allow 3 weeks for the results to be sent to Dr Jen.

*If you already have a DNA report you will need to send the report at time of purchase of your consultation for Dr Jen’s team to confirm it is the type of DNA report that Dr Jen can use.
STEP 3
☐ GET YOUR BLOOD and URINE TESTING BOOKED
Please review the test results that Dr Jen requires.
Tests written in RED are essential,
Tests written in BLACK are ideal but not essential.
Tests written in BLUE are optional.
PLEASE STOP ALL SUPPLEMENTS 4 DAYS BEFORE BLOOD IS DRAWN.
Some tests may not be available in certain countries for example vitamin B5 & B7 are unavailable in Australia.
Blood Tests
- Homocysteine (essential)
- hsCRP (C Reactive Protein)
- Complete Blood Count (CBC) (essential)
- Full thyroid panel (including Reverse T3 and antibodies) (essential)
- Full hormone panel
- Plasma or serum folate (vitamin B9). Avoid RBC folate unless you also corroborate it with plasma or serum. (essential)
- Whole blood Vitamin B1 and 2, vitamin B3 (or NADH/NADPH), Vitamin B5, plasma vitamin B6 (essential), Vitamin B7, and serum vitamin 12 (essential)
- Full cholesterol and Triglycerides (essential)
- HbA1c (essential)
- 1,25(OH) 2 D (essential)
- Full iron panel, serum transferrin, serum magnesium, copper (essential), zinc (essential), sulphur, plasma selenium, 24-hour urine iodine, total glutathione, Uric Acid, Plasma ascorbate, Manganese in whole blood and red blood cells.
- Comprehensive metabolic panel (glucose, calcium, total protein, bilirubin, BUN, creatinine, albumin, sodium, potassium, bicarbonate, chloride)
- Lipid panel
*If you already have recent blood work you will need to send the reports at the time of purchase of your consultation so that Dr Jen can confirm it is the type of labs that she needs.

STEP 4
☐ Submit your lab results
Once you have all of your lab results please send them straight away to team@welladjusted.co
We will confirm receipt of your labs and keep track of all your documents.
STEP 5
☐ Complete Dr Jen’s health history form.

STEP 6
☐ Finalise your Consultation time
Once we have received your history form, Tori who arranges Dr Jen’s appointments will be in touch via email you to arrange your consultation time. Note that Dr Jen is available on Mondays and Thursdays for Consultation (UTC/GMT+1 : Barcelona Time. CET) not including holidays.
